| Green Physical Exam Card |
| Arrowhead requires the actual green physical exam card to be signed and dated by the physician and a parent / guardian.
Per the WIAA rules and regulations, a physical examination taken April 1 and thereafter is valid for the remainder of that school year and the following two school years. A physical examination taken before April 1 is valid only for the remainder of that school year and the following one school year.
ATHLETES CANNOT PARTICIPATE IN AN ATHLETIC SPORT UNTIL THIS CARD IS TURNED IN TO THE
NORTH CAMPUS ACTIVITIES OFFICE
Those 'green cards' can be picked up at the North or South Campus Front Office or the Activities Office at North Campus. Please return completed green cards to the Activities Office. |
